Registered Nurse (RN) | Hospital at Home Location: Avera McKennan Hospital Worker Type: Regular Work Shift: Day/Evening/Weekend/Holiday Shift (United States of America) Pay Range: The pay range for this position is listed below. Actual pay rate dependent upon experience. $32.50 - $48.50 Position Highlights May be eligible for a $15,000 sign on bonus 1-3 years working in a clinical environment. 8am-8:30pm, weekend - every 4th, holiday 2-3 per year, minimal call time. A Brief Overview The Registered Nurse (RN) – Hospital at Home provides acute-level nursing care to patients receiving hospital services in their homes. This role delivers comprehensive, evidence-based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The RN functions as part of an interdisciplinary care team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model.

Responsibilities

Provides patient care by reviewing physician prescriptions; communicating the nursing care plan and all pertinent information to other members of the health care team; coordinating with other healthcare providers; providing physical, emotional, psychological, and spiritual support to patients, families, and friends; conferring with physicians.
Functions as interventionalists in the home utilizing clinical expertise and problem solving to make recommendations to the physicians based on comprehensive physical, psychosocial, emotional, spiritual and bereavement patient assessments.
Develops nursing care plans by obtaining medical histories; examining patients; documenting patient physical condition, psychological status, and influencing social factors; Draw labs; Provide respiratory care; conferring with family and friends; developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions, schedules, and timetables.
