Requirements

• Current Maryland RN license (multi-state licenses welcome)
• CPR & First Aid certification
• Experience in Home Health, Hospice, or Geriatrics preferred
• Strong assessment and clinical decision-making skills
• Ability to work evenings or weekends if needed
• Reliable transportation for home visits
• Knowledge of infection control and patient safety

Nice To Haves

• Experience in Home Health, Hospice, or Geriatrics preferred

Responsibilities

• Lead and support CNAs, GHAs, HHAs, and Med Techs.
• Visit clients at home to assess and check on their health
• Create and update personalized care plans
• Provide skilled nursing and help with medications
• Track client progress and share updates with families
• Connect clients with helpful resources
• Support and guide caregivers
