About The Position

As a Registered Nurse you will be responsible for assessing individual resident’s needs, planning, implementing and evaluating care for a defined group of residents. RNs coordinates the many services that affect residents and their families in the protection and promotion of wellness.

Requirements

  • Currently licensed as a Registered Professional Nurse in the State of Wisconsin or has graduated from and accredited program, carries a temporary permit to function as a Graduate Nurse in the State of Wisconsin and is awaiting State Boards.
  • CPR Certified.
  • Complies with Bethany St. Joseph Corporation mandatory yearly education requirements.
  • Ability to work with all levels of health professionals.
  • On feet majority of time.
  • Ability to lift at least 50 pounds.
  • Ability to bend and stoop frequently.
  • Ability to wash hands in special cleaning solutions.
  • Must have mental capacity and verbal ability to understand written communication and documentation and to communicate with residents, other members of the health care team and relatives.
  • Hearing and visual acuity necessary to communicate.
  • Motor coordination and manual dexterity needed to perform nursing procedures.
  • Ability to tolerate stressful situations.
  • Relatively pleasant, but frequent exposure to unpleasant sights, sounds, orders.
  • Exposure to emotional stress

Responsibilities

  • Maintains a safe and suitable environment for the resident.
  • Assures that all residents’ rights are maintained at all times.
  • Follows all safety, security, infection control and hazardous materials policies and procedures.
  • Integrates our basic nursing philosophy and objectives into resident care.
  • Responsible for assigned unit with direction form RN Charge Nurse.
  • Utilizes the nursing process and Bethany St. Joseph Corporation Standards of Care in the execution of general nursing procedures in the maintenance of health, prevention of illness or care of the ill.
  • Make routine rounds of all residents, noting any changes in condition and reporting to charge nurse.
  • Assesses residents and identify with resident and/or family his/her individual needs.
  • Assesses family/significant other resources as related to individual plan of care.
  • Uses interviews and observations in identification of the resident’s needs and problems.
  • Responsible for documentation of symptoms, reactions, and progress of assigned patients, utilizing electronic medical record as available.
  • Performs venipuncture for blood specimens or intravenous infusions.
  • Initiates, develops, updates, and reviews resident care plans as necessary
  • Reviews resident care and recommended changes to charge nurse
  • Includes resident and/or family in planning care and care conferences
  • Identifies with resident, family, and social worker, resident’s need for discharge planning.
  • Administers medications and treatments as ordered by Provider.
  • Administers direct resident care as necessary.
  • Follows plan of care as documented, re-evaluating as necessary.
  • Communicates resident’s condition changes to the Nurse Manager and or Director of Nursing.
  • Contacts Providers to report changes of condition of resident if charge nurse not available.
  • Fills out accident and incident reports, and reports to provider and families of charge nurse not available and ensures that all resident incidents are documented in the chart.
  • Provides adequate clinical data regarding resident’s condition and progress to other nursing personnel including Director of Nursing via reports PCC.
  • Documents concise current picture of resident’s condition, utilizing the electronic medical record as available.
  • Maintains the inventory of resident's medications on unit.
  • Orders medication and supplies for patients if charge nurse is not available.
  • Contacts the registered Pharmacist regarding resident medications.
  • Accounts for schedule I and II drug inventories as well as drugs in back up.
  • Prepares residents for admissions, appointments, transfers, and discharges.
  • Evaluates and documents response of residents to nursing interventions and physician orders for an assigned group of residents
  • Establishes priorities and adopts/revises in room plan of care
  • Assesses and evaluates residents and/or significant others’ learning needs.
  • Utilizes appropriate resource personnel for assistance with teaching an, when necessary.
  • Assigns ancillary personnel by level and ability to meet residents’ needs.
  • Acts as consultant for LPN Staff Nurse and Nurse Techs
  • Delegates nursing assistant’s activities
  • Communicates plan of care to co-workers
  • Assists in and/or evaluates job performance of LPN and CNAs.
  • Evaluates own nursing practice and participates in resources and/or experiences to enhance learning.
  • Assumes responsibility for knowledge, support, and implementation of current policies, procedures, and guidelines relating to clinical setting. Interprets information to resident’s relatives, physicians and personnel.
  • Accepts constructive criticism, makes appropriate changes, and seek appropriate personnel for advice and consultation.
  • Practices effective communication skills.
  • Participates in problem-solving and offers suggestions for improvement of unit and nursing home function and staff morale.
  • Promotes and maintains effective interpersonal relationships.
  • Consistent read and respond to communications, including emails, ECS messages, and bulletin boards.
  • Participate in Quality Improvement Projects as assigned.
  • Understands that Bethany St Joseph Corporation is a teaching institution and facilitates the educational experience of students affiliating with the nursing home.
  • Exercises informed judgement and uses individual competence and qualifications as criteria in seeking consultation, accepting responsibilities and delegating nursing activities to others.
  • Facilities educational experiences and adjustment of new personnel to nursing home.
  • Takes verbal and telephone orders from providers and transcribes according to policy.
  • Coordinates established restorative and rehabilitation programs, including physical therapy, occupational therapy, and speech therapy.
  • Assists in arranging for spiritual care and religious services.
  • Assists staff to take residents to activities, and make sure that they are properly attired and well supervised.
  • Visits, assists, and observes in as many dining areas as possible during mealtime.
  • Remains cordial to residents, visitors, families and other staff members at all times.
  • Report any repairs needs to charge nurse and then to maintenance department if so directed by charge nurse.
  • Consult with RN charge nurse in handling any emergency situations that may arise.
  • Follows death procedures as directed by charge nurse.
  • Provides leadership to non-professional personnel on the unit.
  • Consult with RN charge nurse regarding staff evaluations.
  • May have to handle staff conflicts and consults with charge nurse.
  • The Director of Nursing is ultimately responsible for the total care of the patient. It is therefore most important that you, the RN and charge nurse work together as a team, so that together maximum care is achieved for each patient.
  • Follows orders as directed by RN charge nurse, Director of Nursing, or Administrator.
  • Performs other duties as directed by Director of Nursing/Charge Nurse.
