Requirements
- Licensed Registered Nurse (RN) in Virginia (or eligible)
- Minimum of 1–2 years of nursing experience required
- Ability to work independently and travel between patient homes
Nice To Haves
- Home health experience preferred, not required
- Knowledge of Medicare/Medicaid guidelines is a plus
- Strong clinical, communication, and organizational skills
Responsibilities
- Provide skilled nursing care in patients’ homes
- Perform patient assessments and develop individualized care plans
- Administer medications and treatments as ordered
- Monitor patient conditions and communicate updates to physicians
- Educate patients and caregivers on disease management
- Complete accurate and timely EMR documentation
- Collaborate with interdisciplinary care teams
