About The Position

This position serves as a central point of contact for our high-risk obstetric patients; guiding them as they navigate complex care pathways, coordinating multidisciplinary resources, and ensuring timely access to information and services. Ideal candidate is able to uphold these responsibilities: Assist patients in understanding their care plans, appointments, and recommended interventions, ensuring clear communication between patients and their healthcare providers. Review and analyze electronic medical records (EMR) to extract essential clinical information, flagging items relevant to ongoing care and facilitating communication among providers. Coordinate follow-up appointments and ensure seamless transitions between care settings. Maintain comprehensive documentation of patient interactions, care plans, and interventions in the EMR. Complete antenatal testing as ordered Ideal candidate profile: Comfortable and confident working with patients experiencing high-risk pregnancies. Highly organized, proactive, and adept at problem-solving. Skilled at synthesizing information from diverse sources, including EMRs and paper records, and translating it into actionable care plans. Committed to promoting patient-centered care and improving outcomes for families facing challenging prenatal journeys. Office hours are Monday - Friday 8am to 4:30pm. Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
