Requirements

Valid RN license in New York State.
2+ years of clinical experience In NY state in home care setting
Knowledge of New York State in-home care regulations.
Strong communication and time management skills.
Reliable transportation and willingness to travel throughout northern Westchester.

Responsibilities

Perform in-home assessments to evaluate client needs.
Document findings and determine appropriate care requirements.
Develop care plans in collaboration with healthcare providers and family members.
Ensure assessments comply with New York State regulations.
Provide education and support to clients and their families
Coach all staff in proper safety procedures
Follow up evaluation of clients to ensure client needs are consistent with the care plan
May include training caregivers on skills assessments
