Requirements

  • Valid RN license in New York State.
  • 2+ years of clinical experience In NY state in home care setting
  • Knowledge of New York State in-home care regulations.
  • Strong communication and time management skills.
  • Reliable transportation and willingness to travel throughout northern Westchester.

Responsibilities

  • Perform in-home assessments to evaluate client needs.
  • Document findings and determine appropriate care requirements.
  • Develop care plans in collaboration with healthcare providers and family members.
  • Ensure assessments comply with New York State regulations.
  • Provide education and support to clients and their families
  • Coach all staff in proper safety procedures
  • Follow up evaluation of clients to ensure client needs are consistent with the care plan
  • May include training caregivers on skills assessments
