As a Registered Nurse (RN), you will make a difference in the lives of patients by providing one-on-one home-based nursing care as directed by an attending physician. You will work at the top of your license, utilizing leadership skills to coordinate care and be part of an interdisciplinary team focused on compassionate quality care and positive outcomes. Home care provides context and real-world perspective about what will truly help patients restore their health. You will work autonomously, managing your time efficiently, and be responsible for all practices and duties within the scope of practice outlined by the state.

Requirements

  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

  • Home care experience preferred.

Responsibilities

  • Provide skilled nursing care in a patient's home as directed by the attending physician.
  • Work autonomously and efficiently manage time.
  • Be responsible for all practices and duties within the scope of practice outlined by the state.
  • Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change.
  • Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.
  • Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
