Requirements
- Active Registered Nurse (RN) license in the state of employment
- 2–3 years of nursing experience preferred (home health experience a plus)
- Strong clinical assessment and critical-thinking skills
- Excellent communication and patient education skills
- Knowledge of home health regulations and standards
- Ability to work independently and manage caseloads
- EHR proficiency
- Valid driver’s license and reliable transportation
Nice To Haves
- Wound care experience preferred
Responsibilities
- Conduct in-home nursing assessments (physical, psychosocial, environmental)
- Develop and implement individualized care plans
- Provide skilled nursing care: wound care, medication management, infusion therapy, catheter care, palliative care
- Monitor patient condition and report changes promptly
- Educate patients and caregivers on disease management and self-care
- Coordinate care with interdisciplinary team members
- Maintain accurate documentation in EHR systems
- Follow infection control and safety standards
- Participate in case conferences and quality improvement activities
