Requirements

  • Active Registered Nurse (RN) license in the state of employment
  • 2–3 years of nursing experience preferred (home health experience a plus)
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and patient education skills
  • Knowledge of home health regulations and standards
  • Ability to work independently and manage caseloads
  • EHR proficiency
  • Valid driver’s license and reliable transportation

Nice To Haves

  • Wound care experience preferred

Responsibilities

  • Conduct in-home nursing assessments (physical, psychosocial, environmental)
  • Develop and implement individualized care plans
  • Provide skilled nursing care: wound care, medication management, infusion therapy, catheter care, palliative care
  • Monitor patient condition and report changes promptly
  • Educate patients and caregivers on disease management and self-care
  • Coordinate care with interdisciplinary team members
  • Maintain accurate documentation in EHR systems
  • Follow infection control and safety standards
  • Participate in case conferences and quality improvement activities
