Check out our website: Home - Fortis Job Summary Select Hospice is seeking skilled and compassionate Hospice Registered Nurses to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.
Requirements
- Graduate of an accredited school of professional nursing (required).
- Current Registered Nurse license in the state of Indiana (required).
- Minimum of 1 year nursing experience in a hospital, facility, or home health setting
- Valid driver’s license, reliable auto insurance, and personal transportation (required)
Nice To Haves
- home health experience (preferred).
Responsibilities
- Perform patient assessments and ongoing evaluations.
- Implement physician-ordered plans of care and complete required documentation (OASIS).
- Provide in-home nursing care for multiple patients per day.
- Educate patients and caregivers on medications, disease processes, and in-home care.
- Coordinate patient services and supervise LPNs/HHAs as needed.
- Communicate changes in patient condition to physicians and care teams.
- Maintain compliance with regulations and agency policies.
