About The Position

JOB SUMMARY: Busy Family Practice looking for a motivated registered Nurse to assist our medical staff. The Nurse will provide support to the health, wellness, and disease management of HealthFirst Patients.

Requirements

Graduate of a Nursing Education Program
Current unrestricted Registered Nursing License for the state of NH
BLS certification required
One year of nursing experience preferred.

Responsibilities

Responsible for nursing care at patient appointments
Obtaining vital signs and patient history
Provides individualized education to patients related to acute and chronic diagnosis
Triages patient calls and schedules visits as appropriate
Administers oral and injectable medication and treatments within the scope of practice and with orders from providers. May distribute sample medication per provider orders.
May perform venipuncture blood draws and urine collection for lab work as ordered by the provider
Maintains patient data and health information in the electronic medical record according to documentation guidelines
Generates patient-related reports, as needed
Participates in performance improvement programs and disease management programs.
Assists in the coordination of well-child and immunization programs
Assists in ordering and stocking clinical supplies
Completes transitional care management follow-up for patients with hospital admissions
Daily monitoring of assigned AthenaOne Buckets for patient follow-up according to outline protocol, including patient portal messages.
Monthly participation on the Life Risk Safety Committee
Routine safety and risk assessment to support Just Culture.
Support care coordination efforts as requested by the primary care provider
