About The Position

TGH Home Care, powered by VNA of Florida, provides a full array of home care services for TGH patients and those in the community needing home care, maintaining the same level of care and experience received at Tampa General Hospital. The Visiting Nurse Association of Florida (VNA) is a 50-year-old and growing home health agency. The Registered Nurse is a qualified professional responsible for the planning, coordination, and assurance of quality care services to patients and their families. This role involves developing work methods and procedures to facilitate high-quality care, delivering professional nursing services in the client's home setting, and providing case management for primary patients. The RN monitors changing client and family needs, provides initial assessments, oversees progress towards goals, and monitors clinical outcomes. They maintain ongoing communication with patients, families, physicians, other disciplines, Clinical Manager, and office staff, and participate in staff Interdisciplinary conferences. In conjunction with the patient and/or their representative, the RN develops, implements, and continuously monitors the plan of care to ensure quality. The position requires completing clinical documentation, including OASIS, evaluations, orders, and care coordination. The RN works with Physicians and Discharge Planners, promotes health and education to patients and caregivers, and functions as a liaison with community health-related resources such as DME, oxygen, and infusions. Interaction with patients, family, team members, and other healthcare professionals is essential, along with participation in ongoing education for continuous personal and professional development.

Requirements

  • Active Registered Nurse License required
  • Must have a minimum of 1-year Clinical Experience

Responsibilities
