About The Position

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) – Wound Care to deliver specialized patient care focused on wound, ostomy, and continence management. This role involves comprehensive assessments, individualized treatment planning, patient education, and collaboration with interdisciplinary teams to ensure high-quality, evidence-based outcomes.

Requirements

• Minimum 2 years of Medical-Surgical (Med/Surg) nursing experience
• Hands-on experience in wound care, ostomy, or continence management
• Associate Degree in Nursing (ASN/ADN) from an accredited program
• Active New York State RN License (in good standing)

Responsibilities

• Perform comprehensive assessments related to wound, ostomy, and continence care
• Develop and implement individualized care plans based on patient needs
• Administer and recommend appropriate treatments, medications, and preventive measures
• Monitor patient progress and document responses to interventions
• Collaborate with interdisciplinary healthcare teams and coordinate referrals as needed
• Educate patients and families on wound care management and prevention strategies
• Support and deliver staff education, including orientation and ongoing training
• Conduct risk assessments for pressure injuries and implement preventive protocols
• Participate in quality improvement initiatives, including data collection and analysis
