About The Position

Requirements

Responsibilities

  • Implement evidence-based plans with patients in coordination with the healthcare team.
  • Develop and modify patient care and treatment plans based on patient responses and conditions.
  • Provide safe and appropriate nursing care and patient education in an ambulatory clinic setting.
  • Collect comprehensive data pertinent to the patient situation and assess the needs of individuals, families, and communities.
  • Communicate findings to patients and appropriate healthcare team members.
  • Partner with patients and the interprofessional healthcare team to review assessments and plan measurable goals and outcomes.
  • Advocate for patients and families in accordance with ANA code ethics and professional practice standards.
  • Support the interdisciplinary healthcare team through supervision, education, and training of other clinic team members.
  • Maintain accurate and detailed reports and records.
  • Prepare patients for and assist with examinations and treatments.
  • Analyze assessment data to determine nursing diagnoses.
  • Direct and supervise less skilled nursing/health care personnel.
  • Promote a positive patient experience.
