About the position

Maxim Healthcare is seeking a compassionate and experienced Registered Nurse (RN) to join our home health care team in Denver. You will provide high-quality care to an adult patient in their own home, making a positive impact on their life. We have patients located in Centennial, Parker, and Lone Tree that are really in need of nursing care for weekend day and night shifts. The schedule can be flexible!

Responsibilities

  • Implement and evaluate individualized care plans for contract care patients, considering their unique needs and goals.
  • Collaborates with healthcare providers to regularly review and adjust care plans to address evolving patient needs and ensure optimal outcomes.
  • Provides comprehensive care that includes therapeutic, preventive, and rehabilitative interventions to promote patient recovery and well-being.
  • Monitors patient's condition closely and promptly reports any significant changes to the appropriate healthcare team members.
  • Ensures accurate and timely medication administration, monitoring treatment effectiveness and documenting outcomes.
  • Continuously assesses care plans and identifies opportunities for patient education, care coordination, goal setting, and discharge planning to support optimal patient outcomes.

Requirements

  • Current Registered Nurse (RN) license in the state of Colorado.
  • Current Health Certificate (as required by facility or state regulations).
  • Current PPD or chest X-ray.
  • Current BLS certification.
  • At least one year of prior RN experience preferred.
