About The Position

Responsible for actively maintaining current network market share and expanding new network opportunities for the Transplant Center throughout South Carolina and the Southeast Region. Serves as the single point of clinical and operational contact for new patient referrals for transplant evaluation and Hepatobiliary (HBP) surgical management. Utilizes registered nurse clinical expertise, transplant knowledge, and care coordination experience to provide education, consultation, and support to Prisma Health physician providers, referring clinicians, patients, and caregivers. Represents the Prisma Health Transplant Center physicians and interdisciplinary care teams, promotes seamless integration of outpatient transplant and HBP services, and supports high quality, patient centered care delivery across the continuum. Implements systems to recruit new patients and providers, optimize patient selection, review clinical outcomes, and provide timely, accurate clinical feedback to referring clinicians. Tracks performance and referral data, conducts regular site visits, and partners with physicians and leadership to develop and execute action plans that improve patient access, safety, and outcomes.

Requirements

  • Associate degree in Nursing.
  • Three (3) years of related healthcare experience.
  • Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

Nice To Haves

  • Bachelor’s degree in Nursing preferred.
  • Experience in transplant field or community outreach preferred.
  • Experience in marketing preferred.

Responsibilities

  • Maintains advanced clinical knowledge of transplant and HBP patient populations, including indications, contraindications, referral criteria, evaluation processes, listing requirements, perioperative considerations, and post‑transplant follow‑up needs.
  • Applies RN clinical judgment to assess referral appropriateness, identify care coordination needs, and escalate clinical concerns to appropriate providers and interdisciplinary team members.
  • Serves as a clinical liaison between referring physicians, advanced practice providers, transplant surgeons, hepatologists, coordinators, and ancillary services to ensure timely, safe, and effective patient transitions.
  • Facilitates cooperative and collaborative relationships among physicians, nurses, advanced practice providers, care coordinators, and operational teams to support high‑quality patient care delivery.
  • Focuses on maintaining existing referral sources and fostering new relationships through clinically informed outreach, education, and consultation.
  • Provides frequent, clinically meaningful feedback to referring clinicians regarding patient status, evaluation progress, listing decisions, outcomes, and follow‑up expectations.
  • Tracks referrals, volumes, conversion rates, and network relationships; analyzes trends and identifies opportunities for growth and quality improvement.
  • Conducts on‑site education and training sessions for physicians, clinicians, and office staff, incorporating nursing perspectives on patient care, safety, and coordination across the transplant continuum.
  • Oversees referral reporting, ensuring accurate and timely receipt, organization, and interpretation of clinical information from referral sites and Prisma Health systems.
  • Disseminates clinical and performance data to physicians, program leadership, and staff; ensures understanding of results through structured meetings, dialogue, and follow‑up.
  • Maintains strong working relationships with peers and colleagues, contributing to a positive, collaborative, and patient‑centered work environment.
  • Utilizes clinical, quality, and performance monitoring tools to support program goals and regulatory requirements.
  • Performs other duties as assigned.
