A transition RN fellow is an RN who has graduated more than 1 year ago, with a minimum of one (1) year of experience practicing as an RN within the last 24 months, but not within an acute care facility. Alternatively, candidates may have acute care experience, but it must be longer than 3 years from the start date. The Transition Fellowship Program helps to refine skills and apply experiences through standardized and evidence-based training learned through practical application of knowledge. It enhances knowledge in a collaborative learning environment, providing the tools, resources, and training for a successful transition into the acute care setting. Additionally, it helps develop professional relationships through mentorship and debrief coaching from accomplished individuals, establishing a support network to guide throughout the career.
