Job Description: Private Duty Registered Nurse You became a nurse to lead care with purpose. At Elara Caring, you guide care where it matters most, in the home. We support more than 60,000 patients each day across the country. As a Private Duty RN, you coordinate care, support families, and lead a team dedicated to one patient at a time. We offer flexibility to fit your life. PRN, Part Time, and Full Time positions are available.

Requirements

• Graduate of an accredited RN program
• Active RN license in the state of practice
• Strong communication and organization skills
• Ability to lift, push, pull, or support up to 50 pounds
• Commitment to patient centered care
• Ability to work within an interdisciplinary team

Nice To Haves

• Two or more years of clinical or home health experience preferred

Responsibilities

• Complete comprehensive admission assessments
• Determine appropriateness for home care services
• Develop individualized plans of care
• Coordinate with referral sources for smooth transitions home
• Communicate with physicians, caregivers, and interdisciplinary team members
• Supervise RNs, LPNs, and Home Health Aides
• Review documentation to ensure alignment with the plan of care
• Reassess and update plans of care based on patient progress
• Coordinate equipment, pharmacy, and community resources
• Ensure quality and continuity of care
