The RN Pt Navigator will assess, address, removes and prevents barriers to care through complex care coordination education, support, and advocacy. This role collaborates with numerous multidisciplinary teams inpatient and ambulatory. Multidisciplinary Team approach to geriatric care within the emergency department to inpatient and post discharge. Responsible for navigation, complex care coordination, case management, extensive patient education, process improvement/development, clinic/ambulatory or specialty clinic knowledge is helpful. Experience in quality improvement or project management and/or research will assist with role. This is a highly collaborative and autonomous role, where great interpersonal and communications skills are of the utmost importance. Prefer experience in geriatrics, family medicine, med/surg. and experience working with elderly and adult population. This is a full time (40 hours/week), salaried, day shift position. The average day starts at 0800 and ends at 1600 and is Monday through Friday. With this role, hours are dependent on patient needs and can start earlier or later on occasion. You will be working with a innovative and dedicated Geriatric Hospitalist team and other specialties to drive positive outcomes for this fragile population. BUMCP ED is designated as a Level 3 Geriatric Emergency Department by the American College of Emergency Physicians' Geriatric Emergency Department Accreditation Program. POSITION SUMMARY This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
