As an RN Patient Navigator, you'll support our Medical Oncology Team. You play a pivotal role in guiding patients through their Oncology Journey. Your expertise encompasses a deep understanding of Oncology treatments, the physical and emotional challenges patients may face. The RN will serve as a trusted resource for patients, providing them with comprehensive education about Oncology, including risk factors, diagnostic procedures, treatment options, and potential side effects. You'll work closely with patients to develop personalized care plans tailored to their specific needs and preferences, ensuring they receive the highest quality of care throughout every stage of their oncology journey. You'll collaborate closely with multidisciplinary healthcare teams, including oncologists, radiologists, and other specialists, to coordinate seamless care and facilitate communication among all involved parties. Your goal is to empower patients to make informed decisions about their treatment and support them in maintaining their overall well-being. Schedule: 4 days per wk. 10hr shifts between Monday - Friday. 6:30a-5pm. Flexibility to start later than 6:30a. Additionally, the campus responsibilities include fully integrated multi-specialty and sub-specialty clinics. POSITION SUMMARY This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
