This full-time Registered Nurse (RN) position within the Patient Access and Care Team (PACT) department is a remote role. The RN will utilize the nursing process and established protocols to telephonically assess patient needs, involving guardians or family when necessary. Key responsibilities include determining appropriate levels of care, providing detailed education, establishing care plans with interventions, and communicating follow-up instructions. The role requires collaboration with on-call providers for escalation, prioritizing patient interactions based on acuity, and applying evidence-based practice to ensure patient safety and reduce risks. The nurse will also assess patient and family readiness to learn, develop strategies to meet learning needs, and support shared governance and quality improvement initiatives. Professional development, effective communication, and ethical decision-making are integral to the role, as is maintaining accurate patient clinical records in the Electronic Medical Record system and communicating patient status to the interdisciplinary team.
