About The Position

The position is for a Licensed Practical Nurse (LPN) at Avera Majestic Bluffs/Avera Sister James Care Center. The role involves the organization and coordination of information to develop and implement individualized, age-appropriate, collaborative plans of care. The LPN will work night shifts from 6:00 PM to 6:30 AM, totaling 48 hours over two weeks. The position may be eligible for a $15,000 sign-on bonus and includes participation in Avera’s Student Loan Repayment Program, which provides additional monthly payments to help pay down student loans.

Requirements

  • An active RN license in the state of practice.
  • Licensed Practical Nurse (LPN) - Board of Nursing.
  • An active license in the state of practice upon hire.

Responsibilities

  • Coordinates staffing to meet the needs of the residents.
  • Delegates and provides follow-up to ensure completion of assigned responsibilities.
  • Provides timely, continuous assessment and preparation of residents and families for the next level of care.
  • Communicates pertinent resident information to the healthcare team, residents, and families clearly and concisely.
  • Documents Medicare assessments and interventions.
  • Files diagnostic and lab reports in medical records following procedures.
  • Transcribes physician orders and rounds with the provider noting orders or data entries.
  • Records and charts clinical data accurately and in detail.
  • Maintains systems of organization, such as monthly ADL, behavior sheets, and BM board.
  • Prepares daily Medicare sheets and lab slips, and charts for physician rounding.
  • Schedules annual medical reassessments, dental, and pain assessments.
  • Recommends positive changes and improvements within the department and facility.
  • Supports a positive living environment for residents and staff.
