About The Position

The Licensed Practical Nurse (LPN) in Long Term Care is accountable for providing each resident with professional nursing care, assessment, and evaluations in their scope of practice. This position involves performing all licensed practical nursing functions and services as directed by the Registered Nurse to all residents. The LPN will complete assessments on residents, make appropriate interventions, apply knowledge, prioritize resident care, and utilize the nursing process (assess, plan, implement, evaluate). The role also includes compliance with pharmaceutical policies and procedures, ensuring that all medications/treatments are given safely and accurately in accordance with the physician’s orders. The LPN participates in the care planning process and assures that resident care is implemented consistently and appropriately to the age of those served. Additionally, the LPN will communicate and manage the delivery of resident care, ensuring quality care is provided, and facilitate conflict management while respecting diversity.

Requirements

  • Licensed Practical Nurse (LPN) - Board of Nursing.
  • An active license in the state of practice upon hire.
  • Ability to work specified hours including night shifts.

Nice To Haves

  • 1-3 years of long-term care nursing services experience.

Responsibilities

  • Perform all licensed practical nursing functions and services as directed by the Registered Nurse.
  • Complete assessments on residents and make appropriate interventions.
  • Apply knowledge and prioritize resident care using the nursing process.
  • Comply with all pharmaceutical policies and procedures.
  • Participate in the care planning process and assure consistent implementation of resident care.
  • Communicate changes in resident condition to physician utilizing SBAR communication tool.
  • Administer medications according to facility policy.
  • Assist with data collection for quality functions and performance improvement activities.
  • Maintain competency with operational equipment and investigate incidents or complaints.
