The Registered Nurse On-Call is responsible for providing after-hours hospice care, ensuring patients receive timely and compassionate support. This role involves planning, organizing, directing, and delivering nursing services in collaboration with the interdisciplinary team. The RN On-Call will develop individualized care plans, address urgent patient needs, and assist families in maintaining comfort and coping during after-hours emergencies. HOURS / SCHEDULE: Full-time, 7 days on / 7 days off (on-call role), Monday through Monday Weekdays (Monday–Friday): On call from 5:00 PM to 8:00 AM Weekends (Saturday–Sunday): On call during daytime hours, with in-person visits conducted from 5:00 PM to 8:00 AM POSITION QUALIFICATIONS Associate degree in nursing from an accredited college or university (or international equivalent) preferred. Minimum of two (2) years of hospice nursing experience required. Current RN license in the assigned state. Current CPR certification is required. Valid driver’s license with a reliable, insured vehicle that meets organizational requirements. RESPONSIBILITIES Ability to work on call night and weekends as needed. Work closely with the sales, clinical, and leadership team. Case manages and provides clinical direction to the interdisciplinary team, physician, and family. Implement/develop/document the patient plan of care. Provide care utilizing infection control measures that protect both the staff and the patient (OSHA). Assure continuity of quality patient care delivered with appropriate documentation. Monitor assigned cases to ensure compliance with requirements of third-party payer. Demonstrate commitment, professional growth, and competency. Promote agency philosophy and administrative policies. Perform on-call responsibilities and provide on-call service to patients/families as assigned. Supervise and provide directions to the home health aide and the LPN. Perform other duties as assigned by management to support department goals and initiatives. SKILLS NEEDED Ability to write, speak, understand, and read the English language. High organizational and work ethic standards. Strong analytical and problem-solving skills. Ability to work independently and as part of a team. Must be computer literate.
