Requirements

Valid and active Registered Nurse (RN) license in the state of practice.
Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
Strong understanding of hospice care principles and compassionate patient interaction.
Excellent communication skills, both verbal and written, to effectively communicate with patients, families, and team members.
Ability to work independently and make sound clinical decisions in high-pressure situations.
Proficient in using electronic health records (EHR) and other healthcare technology tools.
Commitment to uphold the values of compassion, honesty, and patience in all aspects of care.

Responsibilities

Provide compassionate nursing care to hospice patients in accordance with individualized care plans.
Monitor patient health status and record vital signs, symptoms, and any changes in patient condition.
Administer medications and treatments as prescribed by physicians, ensuring adherence to safety protocols.
Collaborate with an interdisciplinary team to coordinate patient care and support services.
Educate patients and families on care processes, symptom management, and end-of-life planning.
Respond to on-call duties promptly, ensuring compassionate and timely care for patients in need.
Participate in quality improvement initiatives to enhance patient outcomes and uphold high standards of care.
