About The Position
The Registered Nurse (RN) position at Hartford HealthCare's Neighborhood Health is a full-time role that requires a commitment to providing professional nursing care to patients in various community settings. As a member of the Neighborhood Health team, the RN will play a crucial role in promoting, protecting, and preserving public health. This involves not only providing direct care but also educating the community about managing chronic conditions and making healthy lifestyle choices. The RN will assess and evaluate community health needs, plan appropriate interventions, and deliver comprehensive nursing services focused on health promotion and illness prevention. The role requires travel to home base hubs in Hartford and New Britain, followed by travel in a company fleet to various clinic sites across Connecticut. The RN will work Monday through Friday, with a standard work schedule of 40 hours per week. In this position, the RN will be responsible for following established standards and practices while delivering care that meets the healthcare, safety, and comfort needs of patients. This includes conducting health assessments, providing education on disease prevention, and implementing interventions based on identified health needs. The RN will also be expected to maintain compliance with all regulatory, state, and company standards, policies, and procedures. The role is integral to the success of the Neighborhood Health initiative, which aims to improve health outcomes in the community through proactive and preventive care.
