About The Position

Leads the education, marketing, and data collection processes, working collaboratively with patients, families, physicians, and nursing staffs. This position will work with the physician practice network to ensure established care plans have been implemented.

Requirements

  • Associate degree in Nursing from an accredited school of Nursing program that is recognized by the South Carolina State Board of Nursing is required.
  • Three (3) years in a clinical setting is required.
  • Registered Nurse in the State of South Carolina.

Responsibilities

  • Work directly with persons with diabetes in Maternal Fetal Medicine and Endocrinology joint clinic to increase patient compliance, uphold system processes, and provide continuous care by overseeing referrals and patient scheduling.
  • Assist in collection/collation of health-related data (e.g. download glucose meter, CGM and insulin pump records).
  • Identify barriers to care such as financial, transportation, missed appointments, gaps in services, poor medication adherence, and facilitate mitigation along with documentation in EH.
  • Liaise with primary care, community hospital and community-based resources.
  • Develop and manage other programmatic areas as assigned, such as telemedicine coordination, scheduling of team planning meetings as well larger grant partner collaborative meetings and participate in multidisciplinary conferences.
  • Continuously assess and improve the department's performance by assisting in the design of operational processes, monitoring performance through data, analyzing data, implementing sustainable performance improvement, and participating in multi-disciplinary inter-departmental PI activities.
  • Accurately complete and document patient vital signs.
  • Report abnormal values to physician.
  • Assist with the continuous assessment and improvement of the department's clinical performance through continuing education, to include new patient and new program development.
  • Assist in the coordination of patient/community education through presentations for the purpose of improved quality of care, patient safety, and clinical compliance.
  • Assist with the management of the environment of care and improved patient quality of care by implementing and monitoring processes to maintaining safe conditions for patients, staff and visitors.
  • Perform other duties as assigned.
