About The Position

Medi-Plex Healthcare Professionals, LLC. is a certified Medicare Home Health Agency that offers comprehensive home health services to its clients. It is important to us that service standards exceed the industry average. Medi-Plex Healthcare is seeking a Registered Nurse (RN) to work within the St. Louis region. We are looking for someone with strong communication skills and dedication to providing quality customer service to our clients. The RN will administer skilled nursing care to patients (in their home). This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.

Requirements

  • Graduate of a state approved school of practical(vocational) Nursing and currently licensed in the state(s) in which practicing.
  • Two (2) years of experience in Nursing preferred.
  • Ability to exercise initiative and independent judgment.
  • No Felony record or State disqualification.
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.
  • Has the ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
  • Has the ability to effectively present information to management, residents, family members, and other employees.
  • Currently Licensed as a RN in the State of Missouri.
  • No Felony record or State disqualification.
  • Ability to calculate figures and amounts such as, proportions, percentages, area, circumference, and volume.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

  • Performs the initial evaluation visit and regularly re-evaluates the patient’s nursing needs.
  • Initiates the plan of care and necessary revisions.
  • As needed, participates in basic patient care, depending on individual patient diagnoses, and type of care given may require addition precautions as noted.
  • Prepare clinical and progress notes for each patient visit and summarize care conference of patients in a timely manner as per Agency policy.
  • Coordinates a variety of services as needed.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Participate, train and prepare for In-service programs as assigned.
  • Process orders and notifies the Physician of patient needs and changes in condition.
  • Complete certification/recertification orders and discharge summaries.
  • Refers to Physical Therapists, Speech Pathologist, Occupational Therapist and Medical Social Worker.
  • Supervises and teaches other nursing personnel.
  • Conducts patient care conferences on patient assigned to his/her care.
  • Ability to be “on-call†as assigned.
  • Access screens, triage and perform treatment ordered for patients
  • Perform medical treatment as ordered by Physician
  • Prepare and dispense prescribed medications
  • Assist with examinations, procedures and other duties
  • Oversee and coordinate the daily patient care
  • Assist patients as needed with walking, transfer, specimen collection, exam preparation, etc.
  • Complete EMR documentation and/or paper-based documentation
  • Other duties a assigned such as new models of care and new types of procedures, etc.
