About The Position

Under the direction of the Clinic RN Manager, the RN provides both home care and clinic nursing functions. In the home care setting, the RN provides home care services to the participants of INSPIRA LIFE. The RN performs initial and annual assessments of participants in their home care environment and coordinates the plan of care with appropriate resources. Reviews plan of care and evaluates participant compliance. The RN provides skilled assessments, treatments, and health education for participants in their homes, according to physician’s order. The RN maintains the participants’ medical records with thorough documentation and communicates participants’ changes and progress to appropriate staff. In the clinic setting, the RN manages the flow of the clinic, leading all staff in the provision of medical and nursing care, ensuring high-quality standards. The RN coordinates services with primary care providers, acting as the primary liaison. The RN triages and assesses participants as needed, provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family, and staff teaching, and communicates the participant’s condition to caregivers, primary care, and the interdisciplinary team. In the absence of the Clinic RN Manager, the RN in the triage setting assumes his/her responsibilities, including oversight of all responsibilities for LPN, Clinical Assistants, and Clinic Clerk. The RN may need to provide position support at another Inspira LIFE Center due to operational need. The RN completes NJ Choice Level of Care (LOC) Assessments initially and annually along with the required QA compliance. As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.

Requirements

  • Graduate of a school of professional nursing, BSN required.
  • Minimum of 1 year working with the frail or elderly.
  • 2 years of Nursing Experience.
  • Active BLS certification required.
  • Active NJ RN License required.
  • Valid Driver’s license, proof of insurance, and reliable means of transportation.

Nice To Haves

  • NJ State Choice Assessment Certification to complete level of care assessments (training provided within the first 12 months of employment).
