Requirements
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse.
- Basic Life Support Certification
- Valid New York State Driver’s License and reliable transportation
Responsibilities
- Deliver and coordinate home health infusion therapy.
- Develop and update a care plan in collaboration with physicians, the patient, and caregivers.
- Educate patients and caregivers on treatment, medication, and health management.
- Ensure interdisciplinary collaboration for holistic care delivery.
