UVA Continuum Home Health Care, the home health and home infusion division of UVA Health, has proudly served the community for more than 30 years. We are currently seeking an experienced and highly skilled Registered Nurse – Liaison to join our interdisciplinary care team. This role requires excellent customer service skills, a strong clinical background, and demonstrated leadership. The Nurse Liaison plays a critical role in ensuring the timely and seamless transition of infusion patients from hospital to home. Serving as the first point of contact for new home health referrals, the Liaison is responsible for clinical coordination, reviewing documentation to ensure appropriateness and readiness for home infusion services, and facilitating effective communication among care teams. The Nurse Liaison works closely with physicians, hospital discharge planners, and interdisciplinary team members to develop individualized plans of care. In addition, this role provides education to patients and caregivers, ensuring they feel confident and supported in managing the IV care required at home.
