About The Position

Our mission is to provide life-changing service with dignity and compassion, helping patients and families navigate a smooth transition home. We’re growing and looking for an experienced Registered Nurse (RN) to join our Hospice Care team! As a Hospice RN, you will plan, organize, and direct hospice care while educating patients and families. You’ll serve as a case manager and ensure quality, compassionate care in the home setting.
Requirements
- Active CA RN License
- Minimum 2 years of nursing experience (hospice or home health experience preferred)
- Strong clinical, communication, and problem-solving skills
- Current CPR certification
- Valid driver’s license, insured vehicle in good condition
- Ability to work independently and manage priorities
Nice To Haves

Responsibilities
- Complete initial and ongoing patient and family assessments
- Develop and revise individualized hospice care plans
- Provide professional nursing care using the nursing process
- Administer medications and treatments as prescribed
- Educate and counsel patients and families on hospice care
- Coordinate care for your assigned caseload
