About The Position

The Hospice Registered Nurse (RN) provides compassionate, patient-centered care to individuals receiving hospice services. The RN is responsible for assessing patient needs, developing and implementing care plans, and providing clinical support to patients and their families. This role focuses on promoting comfort, dignity, and quality of life for patients with life-limiting illnesses. The RN will complete an average of at least five patient visits per day .

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Strong clinical assessment and communication skills.
• Ability to work independently and manage multiple patient visits per day.
• Valid driver’s license and reliable transportation (if travel is required).
• Compassionate and patient-focused approach to care.

Nice To Haves

• Hospice or home health experience preferred.

Responsibilities

• Provide skilled nursing care to hospice patients in accordance with the established plan of care.
• Complete a minimum of five patient visits per day while ensuring quality and compassionate care.
• Perform comprehensive patient assessments and monitor changes in condition.
• Administer medications and treatments as prescribed by the physician.
• Educate patients and family members on disease progression, medications, and end-of-life care.
• Coordinate care with physicians, social workers, chaplains, and other members of the hospice team.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Provide emotional support and guidance to patients and families during the end-of-life process.
• Respond to patient needs and communicate any significant changes to the care team.
• Follow all hospice policies, procedures, and regulatory guidelines.
