About The Position

As a Hospice After Hours Registered Nurse at AccentCare, you will be part of a purpose-driven, collaborative culture that sets the standard for excellence in patient care. This part-time position requires you to work every Saturday and Sunday from 8am to 8pm, providing care in the Anne Arundel and Howard County areas. You will conduct comprehensive nursing assessments, develop individualized care plans, and coordinate care activities to ensure continuity and quality of care for patients. Your role will also involve supervising LPNs, LVNs, and hospice aides, as well as providing education and support to patients and caregivers.

Requirements

  • Registered Nurse (RN) license.
  • Experience in hospice or palliative care preferred.
  • Ability to conduct comprehensive nursing assessments.
  • Strong documentation skills and knowledge of regulatory requirements.
  • Excellent communication and interpersonal skills.

Nice To Haves

  • Experience supervising LPNs, LVNs, and hospice aides.
  • Familiarity with telehealth services.
  • Previous experience in an interdisciplinary team setting.

Responsibilities

  • Conducts comprehensive nursing assessments and develops individualized care plans in coordination with the interdisciplinary team (IDG), addressing physical, emotional, social, and spiritual needs.
  • Maintains timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.
  • Coordinates care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care.
  • Provides education and support to patients and caregivers, and responds to urgent needs during on-call or admissions assignments.
  • Supervises LPNs, LVNs, and hospice aides as needed, and participates in IDG meetings, patient conferences, and ongoing clinical education.
  • Supports pre-hospice services by evaluating patient status via phone, telehealth, or in-person visits and identifying needs for additional care or services.
