About The Position

Tabitha Home Health Care and Hospice, LLC | A Collaboration with Immanuel is seeking a compassionate and skilled PRN Hospice Registered Nurse (RN) to support our Omaha hospice team. This PRN role offers true flexibility — pick up day shifts or weekend shifts based on your availability. This is a great fit for nurses who want meaningful work with the freedom to choose when they serve. As a Hospice RN, you’ll provide supportive, holistic care to clients and families, helping ensure comfort, dignity, and compassion at every stage of the hospice journey.

Requirements

  • AGE REQUIREMENT : At least age 18.
  • EDUCATION : Successful completion of a Registered Nurse course of study.
  • EXPERIENCE : Two years of acute care/long term care nursing experience within the last five years required.
  • CREDENTIALS : RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required.
  • CPR required.
  • License must be kept up to date at the expense of the TEAMember.
  • CPR certification must be kept up to date at the expense of the TEAMember.
  • Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.
  • Must possess auto insurance at or above Nebraska statutory requirements.
  • Must meet Tabitha’s vaccination requirements.

Nice To Haves

  • Bachelor’s degree in Nursing preferred.
  • Experience in community health and/or hospice preferred.

Responsibilities

  • Completes an initial, comprehensive and ongoing comprehensive assessment of Senior/client and family to determine hospice needs.
  • Provides a complete physical assessment and history or current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
  • Assesses and evaluates Senior’s/client’s status by: Writing and initiating plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions Regularly re-evaluating Senior/client and family/caregiver needs Participating in revising the plan of care as necessary, including the Senior/client and the family in the planning process
  • Uses health assessment data to determine nursing diagnosis.
  • Initiates appropriate preventative and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Provides health care instructions to the Senior/client as appropriate per assessment and plan.
  • Assists the Senior/client with the activities of daily living and facilitates the Senior’s/client’s efforts toward self-sufficiency and option comfort care.
  • Acts as Case Manager when assigned by Patient Care Coordinator and assumes responsibility to coordinate Senior/client care for assigned caseload.
  • Completes, maintains and submits accurate and relevant clinical notes regarding Senior’s/client’s condition and care given.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the Senior’s/client’s needs and reports changes in the Senior’s/client’s condition; obtains/receives physicians’ orders as required.
