HOSPICE OF SURRY COUNTYWinston-Salem, NC

About The Position

At Mountain Valley Hospice, what we do is personal—for the patients and families we serve, and for those of us who choose this work. We care for individuals facing serious and terminal illness, providing support that is physical, emotional, social, and spiritual. We also help our communities better understand hospice and end-of-life care. For those called to this work, there is nothing more meaningful. Are you looking for a role where your clinical skill and communication truly matter—where you can help patients and families navigate one of the most important transitions in care? Mountain Valley Hospice is seeking a Registered Nurse (RN) to complete hospice admissions in skilled nursing and assisted living facilities throughout Forsyth County. This role is focused on guiding patients, families, and facility staff through the transition to hospice—bringing clarity, confidence, and compassion to what can often be an emotional and uncertain time. You will work Monday through Friday from 8:00 AM to 5:00 PM, completing hospice admissions in SNF and ALF settings across the Forsyth County service area. You will be working with patients facing serious and terminal illness, and at times may encounter patients who are actively declining or near end of life. This role requires both clinical judgment and the ability to communicate clearly and compassionately in sensitive situations. At Mountain Valley Hospice, we believe hospice is about living well. Our admissions nurses play a critical role in ensuring patients and families receive the right care, at the right time, with clarity and compassion. If you’re looking for a role where your work helps guide patients and families through an important transition, we’d welcome the conversation.
