About The Position

At Phoenix Hospice, Admissions RNs are the first calm presence when hospice care begins. This role involves guiding patients and families from curative care into comfort-focused care with compassion, clinical excellence, and dignity. The position blends strong clinical judgment with deep human connection, addressing not only physical needs but also emotional, spiritual, and psychosocial support. The RN will collaborate closely with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure every admission is smooth, thoughtful, and centered on what matters most.

Requirements

• Active Kansas or Compact RN license
• Strong assessment, organization, and communication skills
• Ability to work independently while collaborating with a team
• Valid driver’s license

Nice To Haves

• Hospice experience preferred

Responsibilities

• Complete hospice admissions and skilled nursing visits
• Build meaningful, day-to-day connections with patients and families
• Provide education on conditions such as CHF, COPD, and Diabetes
• Perform wound care and medication management
• Change dressings and medical devices as ordered
• Accurately document patient conditions and communicate changes to physicians
• Partner closely with interdisciplinary hospice teams to deliver seamless care
