LV HOME CARE AND HOSPICE HOSPICE ADMISSIONS NURSE - REGISTERED NURSE (RN) WEEKEND DAYS Summary Responsible for determining hospice as the appropriate level of care for the patient based on hospice Conditions of Participation. Performs initial and follow up assessments, completes hospice holistic assessment as per CMS regulation, and formulates an individualized plan of care according to physician orders that incorporates the analysis of assessment data and current scientific findings. Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient/family centered approach and provides patient/caregiver education aimed at achieving patient goals/outcomes.

  • Performs comprehensive subjective and objective initial and follow up assessments of patient's status that includes physical, psychological and environmental parameters.
  • Responsible for completing the hospice comprehensive assessment as per CMS regulation with a high level of accuracy that reflects quality outcomes measures and appropriate financial reimbursement for services.
  • Formulates an individualized plan of care according to physician orders that incorporates the analysis of assessment data and current scientific findings.
  • Relays significant changes in patient status to the physician and other members of the interdisciplinary care team in a time period consistent with patient needs.
  • Collaborates with the physician and hospice interdisciplinary team.
  • Determines the appropriate hospice needs and level of care based on the hospice Conditions of Participation.

  • Provides direct nursing care with the emphasis on end-of-life support, pain control and other symptom managment.
  • Creates an environment of caring that is welcoming and nurturing to new employees.
  • Supports the culture of safety and complies with the organizations established policies and procedures.
  • Participates in service recovery as necessary.
  • Actively participates in the orientation of new staff.
  • Bachelor’s Degree Nursing or Associate’s Degree Nursing with 1 year experience in the hospice setting
  • 2 years Experience as a registered nurse in acute care or hospice/ home health care setting. or 1 year Recent experience in hospice nursing
  • Knowledge of patient education techniques and principles
  • Ability to take initiative and function independently without direct supervision and as a member of the care team
  • Ability to travel to patient care assignments
  • Knowledge of Hospice Conditions of Participation
  • Ability to facilitate difficult and complex conversations related to end-of-life care,
  • Willingness to travel within the defined service area to meet both patient and the business needs of the organization.
  • BLS - Basic Life Support AHA - American Heart Association Upon Hire

  • Bachelor’s Degree Nursing
  • CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years
