About The Position

The position involves assessing the patient's physiologic and psychosocial health status, conducting patient interviews, and explaining policies and procedures to patients and their significant others. The nurse will review patient charts, answer questions, and assess various health statuses including gastrointestinal, cardiovascular, respiratory, renal, and neurological. The role also includes determining mobility, sensory deficits, and skin conditions, as well as assessing pain levels and managing pain effectively. The nurse will communicate and document the patient's health status and care plan, develop criteria for measuring patient outcomes, and identify actual or potential patient problems. The nurse will formulate and implement a plan of care, ensuring safety precautions are followed, and provide emotional support to patients and their families. Additionally, the nurse will evaluate the care provided, collaborate with other care team members, administer medications, and participate in patient and family education.

Requirements

  • Graduate from an accredited School of Nursing.
  • Bachelor’s Degree in Nursing preferred.
  • Must be enrolled in an accredited program within 24 months of employment if hired after September 1, 2010.
  • Obtain a BSN Degree within five (5) years of employment date.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • BCLS, ACLS preferred.

Responsibilities

  • Assesses the patient’s physiologic health status.
  • Conducts patient interviews and explains policies and procedures.
  • Reviews patient’s chart and answers questions correctly and courteously.
  • Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
  • Determines mobility, sensory deficits, prostheses use, and skin condition.
  • Assesses level of pain and pain management.
  • Communicates and documents patient’s physiologic health status and plan of care.
  • Assesses patient’s psychosocial health status.
  • Elicits perception of medical/nursing care and expectations.
  • Determines coping mechanisms and knowledge level.
  • Identifies cultural/ethnic requirements to reduce anxiety.
  • Communicates and documents psychological status and care plan.
  • Develops criteria for measurement of patient outcomes.
  • Identifies actual/potential patient problems.
  • Identifies patient’s need for teaching based on psychosocial and developmental assessment.
  • Develops patient outcome statements and establishes individualized patient goals.
  • Identifies care activities and establishes priorities to achieve expected outcomes.
  • Coordinates the cost-effective use of supplies, equipment, and medication.
  • Documents the plan of care and collaborates with health team members.
  • Maintains constant vigilance over patient care to ensure safety.
  • Exercises professional skills related to the plan of care.
  • Reassesses patients as needed and revises the plan of care.
  • Correctly administers prescribed treatments and uses necessary equipment.
  • Provides emotional support to patients and significant others.
  • Applies scientific principles in performing procedures safely and efficiently.
  • Makes accurate observations of patient’s conditions and documents appropriately.
  • Acts rapidly and effectively during emergency situations.
  • Participates in planning for discharge and coordinates referrals.
  • Provides discharge instructions to patients and significant others.
  • Demonstrates ability to measure effectiveness of care provided.
  • Performs variance analysis related to outcome data for performance improvement.
  • Collaborates with other care team members in planning and carrying out treatment regimens.
  • Assists the Physician during treatments and/or diagnostic procedures.
  • Uses clinical judgment in delegating assignments to care team members.
  • Performs grade I-IV Decubitus Care and preventive skin care measures.
  • Administers medications correctly and safely.
  • Educates patients and significant others related to drug and food interactions.
  • Provides explanations of conditions to patients.
  • Recognizes and utilizes health teaching opportunities.
  • Evaluates the effectiveness of teaching by feedback from patients/families.
