Requirements

• Current RN licensure in state of practice.
• Current CPR certification.
• Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with other healthcare professionals involved in patient care.
• Make initial and comprehensive nursing evaluation visits to determine the primary focus of care.
• Develop and submit accurate, complete, and timely documentation in collaboration with physician orders.
• Perform patient comprehensive assessments at designated time points and develop appropriate plans of care.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
