Requirements

• Active RN License in Pennsylvania
• Minimum 1 year Home Health experience
• CPR Certification
• Reliable transportation & valid driver’s license
• A compassionate, dependable nurse who takes pride in their work

Responsibilities

• Deliver one-on-one skilled nursing care in patient homes
• Perform initial assessments and ongoing evaluations
• Develop & update individualized care plans
• Collaborate with physicians, families, and caregivers
• Educate patients on managing their health
• Complete documentation accurately and on time
