Registered Nurse (RN) Home Health - South Bay
About The Position

Local Territory Only: Your patients are in Santa Clara County / South Bay. You will never be asked to drive to San Francisco or the East Bay. Support, Not Stress: Our back office is staffed by licensed clinicians who know exactly what you are going through. Need to call a doctor? Running late? We handle the logistics so you can focus on the patient. Quality Over Quantity: We have low production goals because we believe excellent care takes time. You won't be rushing from door to door. Financial Package We offer the stability of a salary with the upside of a performance bonus. Annual Salary: Starts at $135,000 per year (Increases based on experience & Case Management skills) Performance Bonus: Earn an additional up to 25% of your annual wage based on quality-of-care metrics. Overtime Eligible: Unlike many agencies, our clinicians are NOT Exempt. If you work late or take an extra shift, you get paid for it. Sign-On Bonus: $10,000 available for qualified candidates. Requirements
- Current California RN License.
- 1+ year of nursing experience (Home Health preferred).
- Valid Driver's License and reliable transportation.
- Compassionate, self-driven, and ready to work independently.
Responsibilities
- Provide skilled nursing care in accordance with the patient's individual Plan of Care.
- Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes.
- Supervise Licensed Vocational Nurses (LVNs) when assigned.
- Evaluate patient progress and report changes to the Case Manager or Director of Nursing.
- Complete documentation in a timely manner using our AI-powered charting software.
