We specialize in home health care services that enable the patient to adapt and manage his or her medical situation in the comfort of their home. Stella Maris offers home health services to residents of Baltimore City, Baltimore County, and Harford County. Stella Maris is looking for Home Health RN Case Managers to join their growing Home Health team. This is an exciting opportunity to work alongside dedicated clinicians to meet the needs of clients in the community we serve. RN-Case Manager (Home Health) Full-time Monday-Friday Home Health Nurse-Registered Nurse Stella Maris Voted #1 “Best Homecare Services” for 2023 by Baltimore Style Magazine DESCRIPTION: Under the direction of the clinical manager, the RN Home Health Case Manager performs primary nursing care functions in conformance with professional standards of home health nursing practice. Providing individualized care to patients in their home under the plan of care approved by the primary physician. The home health nurse coordinates all services provided for each assigned patient and family. The Case Manager provides leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources. The Case Manager manages clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care. This position requires daily travel in assigned region #INDHP

Requirements

• RN licensure in the State of Maryland required
• Prior experience in hospice or home health strongly preferred
• Valid driver’s license and reliable transportation
