Buena Vista Home Health is seeking a Registered Nurse (RN) to join their Home Health team. The Home Health Registered Nurse (RN) plans, organizes, and directs home health services while providing high-quality, patient-centered care, collaborating with an interdisciplinary team to develop individualized care plans and deliver skilled nursing services in the patient’s home. Requirements

  • Active California RN license
  • At least 2 years of nursing experience
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle
  • Strong clinical judgment, communication, and documentation skills
  • Ability to work independently and manage time effectively
  • Able to lift, transfer, and position patients as needed
  • Comfortable with walking, standing, bending, and other physical activity required in home care

Nice To Haves

  • Experience in home health or public health (as part of the 2 years nursing experience)

Responsibilities

  • Develop, implement, and revise individualized plans of care with patients, caregivers, and providers
  • Provide skilled nursing care, including preventive and rehabilitative services per physician orders
  • Deliver patient and caregiver education to promote independence and safety
  • Offer counseling and support to patients and families
  • Maintain accurate and timely clinical notes and documentation
  • Communicate regularly with physicians and healthcare team members regarding patient status
  • Identify and coordinate discharge planning needs
  • Ensure care is delivered in compliance with all regulatory and company standards
