Compensation Package: Competitive piece-rate pay with multiple bonus opportunities: Per Visit: $70 – $130 per completed visit (good-faith estimate; varies by visit type) Drive Time: $25 per hour Mileage Reimbursement: $0.72 per mile Productivity Bonus: Earn additional incentives based on performance Documentation Bonus: Reward for timely submission of documentation Benefits Flexible scheduling Opportunities for specialization and career advancement Supportive team environment Mileage reimbursement and drive-time pay JOB SUMMARY The registered nurse plans, organizes and directs/coordinates home health services and is experienced in nursing, with emphasis on community health education. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Requirements

  • Valid CA Registered Nurse License
  • 2+ years of nursing experience, home health experience preferred
  • Maintains a current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Responsibilities

  • Develop and initiate the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s)
  • Initiate proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care
  • Provide patient and caregiver education
  • Identify discharge planning needs as part of the care plan development and implements prior to discharge of the patient
  • Ensure that arrangements for equipment and other necessary items and services are available
  • Supervise Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)
