SummitWest Care is hiring a Full-Time Registered Nurse (RN) for our home health team. Requirements
- Active Colorado Registered Nurse (RN) license
- Strong clinical assessment and critical thinking skills
- Ability to work independently while managing a structured caseload
- Clear, professional communication style
- Comfort with documentation and basic technology
- Reliable vehicle, valid driver's license, and auto insurance
Nice To Haves
- Home health experience preferred (we will train the right candidate)
Responsibilities
- Provide skilled nursing care in a home health setting
- Complete comprehensive patient assessments and develop individualized plans of care
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on medications, treatments, and self-care
- Coordinate care with an interdisciplinary team (LPNs, therapists, caregivers)
- Complete documentation in Homecare Homebase (HCHB) – training provided
- Participate in a rotating on-call schedule
- Travel locally within Grand Junction, Fruita, Clifton, and Palisade
