We are looking to hire a Registered Nurse PRN to join our clinical field team for home visits. Requirements

  • Graduate of an accredited Registered Nurse educational program
  • Current Registered Nurse (RN) license within the state of practice
  • Minimum of one (1) year of Registered Nurse experience in home health, hospice, hospital or skilled nursing facility
  • Current CPR certification
  • Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
  • Strong initiative, follow through, process orientation, attention-to-detail
  • Reliable transportation, valid driver’s license and current automobile insurance

Nice To Haves

  • previous home healthcare /OASIS data assessment tool experience preferred

Responsibilities

  • Conducting one-on-one patient care and patient / family teaching in the home setting.
  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Regular communications with our clinical management team and timely and accurate documentation.
