About The Position

The Registered Nurse (RN) position at Lutheran SeniorLife involves providing skilled nursing care and management of patients in their homes, particularly focusing on palliative and hospice services. Requirements

  • Currently licensed as a Registered Nurse in the Commonwealth of PA.
  • One year of experience in hospital medical/surgical nursing, public health nursing, or related experience preferred.
  • Prior home health care, hospice, psychiatric, and/or mental health nursing experience preferred.
  • Dependable transportation with a valid driver's license is required.
  • Working knowledge of computers, Microsoft Word, and Excel is required.

Nice To Haves

  • Experience in hospice or palliative care nursing.
  • Experience in psychiatric or mental health nursing.

Responsibilities

  • Provide all aspects of skilled nursing care and management of patients.
  • Supervise aides and provide guidance to LPNs regarding patient care.
  • Report admission information and changes in the patient's condition to the Patient Care Coordinator (PCC).
  • Implement changes to the plan of care as discussed with the PCC.
  • Provide timely and appropriate patient/family education and support based on diagnosis and expected outcomes.
  • Identify needs for other internal and external patient care services and make appropriate referrals.
  • Coordinate the overall care provided to the patient.
  • Participate in quality assessment and improvement activities.
  • Support team members in delivering care and fulfilling daily responsibilities.
