About The Position

Part-time RN needed for client assessments for COMFORCARE Home Care. Are you a compassionate Registered Nurse (RN) looking for a flexible schedule and meaningful one-on-one care experience? ComForCare Home Care in Baltimore, MD is hiring RN Case Managers to conduct in-home assessments and provide ongoing client care and case management. We have immediate openings with flexible hours, including weekdays, evenings, and weekend shifts.

Requirements

• Current Maryland Registered Nurse (RN) license or multi-state licensure
• CPR/First Aid Certification
• Experience in Home Health, Hospice, and/or Geriatric Nursing preferred
• Strong assessment and clinical decision-making skills
• Ability to work evening and weekend shifts, as needed
• Reliable transportation for home visits
• Familiarity with infection control and patient safety procedures

Responsibilities

• Conduct comprehensive in-home assessments and reassessments
• Create and revise individualized care plans based on clinical findings
• Provide medication administration and skilled nursing interventions
• Monitor and document changes in client health and status
• Communicate regularly with clients, caregivers, and care teams
• Initiate referrals to appropriate healthcare or community resources
• Maintain timely, accurate documentation in accordance with regulations
• Participate and facilitate caregiver in-service programs
