About The Position

The position involves providing comprehensive nursing care in a home health and hospice setting. Responsibilities include maintaining professional knowledge, assuring quality of care, protecting patients and employees, and establishing rapport with patients. The role also emphasizes the importance of emotional support, patient independence, and documentation of care services. The nurse will supervise Home Health Aides, resolve patient needs through teamwork, and contribute to the overall quality of care. Additionally, the nurse will be involved in hospice care, assessing eligibility, educating patients and caregivers, and implementing mental health concepts as needed.

Requirements

  • Graduate of an accredited Nursing Program required.
  • Six (6) months medical/surgical experience.
  • Strong leadership ability and good organizational skills.
  • Independent and critical thinking skills.
  • Strong ability to communicate complex topics to diverse audiences.

Nice To Haves

  • Bachelor's Degree in Nursing preferred.
  • One (1) year experience in medical/surgical nursing or healthcare related field.
  • IV and phlebotomy training.
  • Certification in an applicable area.
  • Prior Hospice and/or oncology experience.
  • Hospice and Palliative Care Certification.
  • Experience in a Healthcare setting.

Responsibilities

  • Maintain professional and technical knowledge by attending education workshops and participating in professional societies.
  • Assure quality of care by adhering to therapeutic standards and following nursing division's philosophies.
  • Protect patients and employees by adhering to infection-control policies and medication regulations.
  • Maintain patient confidence by keeping information confidential.
  • Identify patient care requirements by establishing rapport with patients.
  • Establish a compassionate environment by providing emotional and psychological support.
  • Promote patient's independence by teaching self-care skills and answering questions.
  • Document patient care services by charting in records.
  • Maintain continuity among nursing teams by documenting and communicating needs.
  • Maintain a safe and clean working environment by complying with regulations.
  • Maintain nursing supplies inventory and place orders as needed.
  • Provide direct and indirect supervision of personal care provided by Home Health Aides.
  • Resolve patient problems using multidisciplinary team strategies.
  • Maintain cooperative relationships among healthcare teams.
  • Contribute to team efforts by accomplishing related results.
  • Understand and perform care according to the Hospice philosophy.
  • Assess eligibility for Hospice care and obtain informed consent.
  • Function as a primary nurse in the Interdisciplinary Group.
  • Follow guidelines for pain and symptom management.
  • Educate patients and caregivers regarding end-of-life care needs.
  • Implement mental health concepts in the plan of care.
  • Provide supportive psychotherapy.
  • Make appropriate referrals based on nursing assessments.
  • Function as a resource for Agency staff with mental health problems.
  • Perform other duties as assigned.
