About The Position

Responsibilities

  • Conduct initial and ongoing patient assessment in collaboration with the interdisciplinary care team.
  • Analyze assessment data and create a plan of care.
  • Implement treatment and evaluate treatment effectiveness.
  • Administer medications in a safe manner consistent with the State of Practice and organization policies and procedures.
  • Serve as point of contact for patients' care coordination throughout hospital departments.
  • Act as a patient safety advocate by participating in ongoing quality improvement in the department.
  • Incorporate the professional practice model into care delivery.
